NURSING HOME TRANSITION SPECIALIST:
Full-time (40 hour) professional position to identify and assist people with disabilities move from institutions to community based settings.
Must be a self-starter who will work professionally with consumers, family members, other human service providers, and funding sources.
Bachelor's degree and own transportation required.
Must have a minimum of one year prior experience working with people with disabilities.
